How to create a survey in Microsoft Teams

When you want a questionnaire

Use Microsoft Forms. It is already in Microsoft 365. Microsoft’s own check-in guide is Survey your team to check in and stay connected.

1. Open Forms from the Microsoft 365 app launcher, or from a Teams tab.
2. Create a new form. Add the questions you need.
3. Share the link in a channel, pin Forms as a tab, or collect responses in a meeting.
4. Read responses in Forms. Names are recorded unless you turn that off in settings.

That path is right for onboarding questionnaires, longer pulse forms, and anything that does not need to be answered on a chat card in thirty seconds.

When you want a survey in the conversation

A lot of “survey in Teams” jobs are “ask the channel, get answers before standup ends.” Forms sends people out of the thread. Chalk Polls & Surveys keeps the questions on an Adaptive Card.

Anonymous answers
