Get it from AppSource or search "Chalk Polls" in the Teams app store. Install walkthrough.
Open compose or the Polls tab. One multiple-choice question is a poll; several questions or open text is a survey. Need it faster? @-mention the bot for a single poll.
Responses stream in live. Results update in the conversation — no extra dashboards needed.

That is a survey: 1 to 10 questions, mixing multiple choice with open text. Mark each one required or optional. Give it a title when there is more than one question.
People answer in a stepped dialog (Next, Back, Submit). The channel card shows the title and how many people responded, not option bars. Bot and Copilot still create a single multiple-choice question.
